Professional Skilled Nursing Assessment Care / Case Management
As a public health agency, we do provide health maintenance-type visits in the city and the county. Clients are accepted into the program based on the reasonable expectation that their medical, nursing, and social needs can be adequately met in their place of residence. Accepted clients:

• Are under the care of a physician
• Have an identified need that requires nursing services provided in the home to restore, improve, or maintain their health
• Do not require in-home nursing as defined by Medicare
• Do not require nursing visits more than once a week
• Have a reasonable expectation that their needs can be safely met in their home
• Reside in Burleigh County
• Are assessed a fee based on a sliding scale

Referrals & Follow-Ups
You may know someone who could benefit from our services. Potential clients could:

• Benefit from nursing visits
• Benefit from medication set-up and medication compliance assessment
• Use an advocate for them with their physicians
